47-Year-Old Gets Relief From Double Vision and Droopy Eyelid From AEHI

47-year-old John Charles presented himself at Advanced Eye Hospital Institute (AEHI) located at Sanpada, Navi Mumbai as he was experiencing droopy eyelids, occasional double vision and mild dizziness for four months.

John was referred to Dr. Akshay Nair, an Oculoplastic Surgeon specialising in treatment of the eyelid disorders at AEHI. Detailed eye check-up revealed that John had fluctuating droopy eyelids (ptosis). His eye motility was also reduced.

Ice pack test was performed and was found to be positive. The ice pack test is a safe, inexpensive, and quick to perform procedure. In this test, a covered pack of ice is applied over the closed  eyes on the eyelids for 2 minutes. If the patient shows improvement in the diplopia or in the level  of the eyelids by more than 2mm after the ice pack is removed, the result is deemed to be positive.
 Additionally, double vision evaluation through diplopia charting was carried out.

Double Vision

Diplopia chart is the record of subjective separation of double images in the nine positions of gaze. This test helps to understand if the diplopia is more for near or distant objects, predominantly vertical or horizontal gaze etc.

Further, John was advised to get acetylchloline antibody titres done which were found to be elevated. Elevated AChR-Ab titers confirm the diagnosis of MG. However, a normal titer does not exclude the disease. AChRAb testing also provides a baseline for future comparisons and response to immunomodulatory treatment.  

Dr. Nair then diagnosed with him with Ocular Myasthenia and asked him to consult a neurologist as well to identify any systemic acquired cause for MG.   

He was started on some oral tablets and asked to review back after a month. On the follow-up, he showed good improvement in lid elevation with mild residual droopiness. Further, his eye motility also showed improvement. Dr. Akshay then advised him to continue his tablet aimed to decrease the muscle weakness from myasthenia gravis.
