Delaying Treatment for Retinal Diseases Can Lead to Retinal Detachment

44-year-old Ramesh Verma (patient’s name changed to protect his identity) had high myopia for which he had undergone radial keratotomy in 1995. He presented for general check-up at Advanced Eye Hospital Institute (AEHI) located at Sanpada, Navi Mumbai.

Thorough examination showed that, with glass, the visual acuity was 6/18 in the right eye and 6/12 in the left. Retina examination revealed he had lattice regeneration with holes because of high myopia bilaterally. Additionally his right eye showed a retinal tear with a cuff of fluid around it.

Dr. Yogesh, Vitreo-retina specialist recommended him barrage laser treatment as delaying this treatment can increase the risk of retinal detachment. Ramesh decided to go for barrage laser later and went away with his glass prescription.

However, Ramesh visited AEHI directly after three weeks with the complaints of sudden reduction of vision in the right eye. Eye check-up revealed Ramesh’s right eye vision had deteriorated to finger counting to 1 meter. There was subtotal retinal detachment, meaning fluid from the vitreous cavity was flowing through the retinal hole under the retina forcing it to detach from its position. When the retinal detachment extends centrally it can impair the vision dramatically.

Retinal detachment:- It is an eye disorder in which the retina (light-sensitive tissue at the back of the eye) separates from the layer underneath. Retinal detachment, if not treated on time, can lead to permanent loss of vision.

With this unpleasant advancement of retinal detachment, Ramesh underwent Vitrectomy with silicon oil insertion with retinal buckling with barrage laser.

Silicone oil is of immense significance as a tool during vitreoretinal surgery. Silicone oil helps in maintaining the adhesion between retina and retinal pigment epithelium (RPE).

And after one week post-operation, his uncorrected vision improved to 6/60 and after 4 months it further improved to 6/24. Although, Ramesh regrets his decision to delay the barrage laser treatment, he is relieved to have vision.

•    Most retinal diseases, which directly hampers vision, are non-regenerative  in nature.
•    Hence, delaying treatment and/or surgery almost guarantees blindness.
•    Therefore, early diagnosis and timely treatment is of vital importance.
