Blurry Vision After Cataract Surgery?

75-year-old Laxmibai Deshpande* had watery eyes and was experiencing hazy vision since a year. So, she presented herself to Advanced Eye Hospital & Institute (AEHI) in Sanpada, Navi Mumbai.

At AEHI, Visual acuity test confirmed that her vision had reduced to 6/60 in left eye and 6/18 in right eye. This is because both her eyes had developed posterior capsular opacity (PCO). Meaning cloudiness on the lens capsule. Laxmibai’s family was not happy to hear this as she had undergone cataract surgery few years ago from a local eye hospital.

Dr. Vinayak Damgude, an eye doctor for Cataract at AEHI, recommended Laxmibai to undergo YAG capsulotomy in both eyes.

YAG Laser capsulotomy is a simple, safe and quick laser treatment. This procedure is aimed to treat posterior capsular opacity (PCO) after cataract surgery. YAG Laser capsulotomy creates a small hole in the opacified capsule. This small hole allows the light to pass through.

Eventually, YAG capsulotomy was carried out with Laxmibai’s consent.

PCO is a common occurrence after previous cataract surgery and can be treated quickly with laser treatment.

Once the hazy lens is removed by cataract surgery, it does not come back. During the cataract surgery intraocular lens is placed on the remaining capsule of the lens. Over time few (epithelial) cells of the lens capsule grow on the capsule that makes it thicker and less transparent. This gives cloudy vision to the patient. PCO can develop in some people after months or years after the previous cataract surgery. It is also called as after cataract. PCO can obstruct vision and make the vision blurry the severity of which is dependent on the type and thickness of the lens capsule.

After Yag capsulotomy, Laxmibai’s vision got restored to 6/9. She was advised to instil an eye drop for a week following the laser.

The occurrence of PCO is quite common. Following factors increases its likelihood.

  • Cataract surgery at a younger age.
  • Patients with previous eye inflammation or retinitis pigmentosa.

Nevertheless, it is also possible that patients, who do not have any pre-existing risk factors may also develop PCO. Many factors impact it and most people develop it after cataract surgery.
