Kamothe Man Receives Best Cataract Surgery After Retina Hole Treatment at AEHI

42-year-old Vinay Ahuja experienced eyestrain and intermittent blurring of vision. Therefore, he presented himself to Advanced Eye Hospital & Institute (AEHI) located at Sanpada to consult the best eye specialist in Navi Mumbai as per the google reviews.

Vinay was referred to Dr. Yogesh Patil, a Vitreo-retina specialist at AEHI. He complained to Dr. Yogesh that  he was experiencing intermittent blurring of vision.  Detailed eye examination revealed that he had high myopia with presbyopia and Ectopia lentis with zonular stretching in both his eyes.
Dr. Patil also observed that there was a retinal hole in his right eye in the infero temporal quadrant. Dr. Yogesh then recommended barrage laser treatment to seal the retinal hole.

Barrage laser treatment is an OPD procedure, meaning treatment is a few minutes procedure. It is sealed using a laser like a welding technique.

After a week, Vinay came for follow up wherein it was found that the retina was well barraged. .

Ectopia lentis is an eye condition where the natural crystalline lens is dislocated or displaced from its natural position. Subluxated lens is partial dislocation and complete malposition of lens is referred to luxation of lens. Subluxation of lens may occur from congenital or developmental conditions such as Marfan syndrome, homocystinuria.

Further, Vinay was advised to undergo test to check homocysteine levels in his blood.

Homocystinuria is an amino acid (methionine) metabolism disorder wherein homocysteine is accumulated abnormally in blood, which can be very toxic to the whole body as it causes damage to nervous system.

Vinay was prescribed with new glasses for his high myopia and asked to follow up after six months.

Post six months, eye examination showed that his lens had developed Nuclear Sclerosis Grade I with Compound Myopic Astigmatism. He was then asked to follow up after six months.

However, Vinay visited AEHI after a year with the complaint of blurring of vision and consulted Dr. Vandana Jain, who is Cataract, Cornea, LASIK surgeon at AEHI.

Dr. Jain observed Vinay’s eyes through slit lamp and found that his lens had developed Nuclear Sclerosis Grade II i.e. cataract. She recommended him to undergo Phacoemulsification with TORIC intraocular lens to correct the astigmatism along with capsular tension ring (CTR) implantation to support the subluxation ( as it was only 2-3 clock hours).

Distance between the front and back poles of your eye is termed as axial length and it is measured by a test known as Biometry. His lens power was calculated by IOL MASTER and with Corneal Topography his cornea surface was mapped.

Phacoemulsification was carried out subsequently. The recovery time for vision improvement is generally a few days however; patient is expected to observe some precaution for a few weeks after the cataract surgery so as not to develop any side effects or complications after the surgery. Most people can go back to work after cataract surgery in 3-4 days.

To manage a cataract patient with pre-existing weak zonules or patient with subluxated lens necessitates expert knowledge from the cataract surgeon.

Capsular tension rings (CTRs) are C-shaped devices which can stabilize the capsular bag of the crystalline lens during cataract surgery in eyes with zonular weakness. This is commonly seen in patients with high myopia like Vinay, eye trauma cases, previous ocular surgery (eg, vitrectomized eyes), etc.

  • Lens dislocation can occur since birth or may develop due to high homocysteine levels, Marfan’s syndrome, low sulphate oxidising enzyme, eye trauma, hyperlysinemia (high lysine levels in blood) etc.
  • It is not a wise decision to delay an eye check up or subsequent follow ups once detected to have ectopia lentis
  • Ectopia lentis can be progressive
  • It can be treated with advanced tools like capsular tension rings etc.
