Kharghar resident avails best laser treatment for impaired vision at Advanced Eye Hospital
42-year-old Mr George Mathew (patient’s name changed to protect his identity) was seeing black spots in front of his right eye since 3 weeks. This complaint gradually increased over last week, which made his near visual activities like office work and reading more difficult. So, he presented with these eye problems along with strain in his right eye at Advanced Eye Hospital Institute (AEHI) located at Sanpada, Navi Mumbai.
Detailed eye check-up was conducted for him. The visual acuity was 6/18 N18 (haziness) in the right eye and 6/6 N6 (normal) in the left. His intraocular pressure were normal i.e. 12mm Hg and 11mm Hg in right and left eye.
Based on Mr. Mathew’s eye complaints, he was referred to Dr. Yogesh, Vitreo-retina specialist at AEHI. Dr. Yogesh recommended him to undergo Macular OCT (Scan for central part of Retina) for both his eyes. This examination revealed swelling (pockets of subretinal fluid) at the macula in the right eye while the left eye’s OCT was normal.

Dr. Yogesh Patil diagnosed George with Central serous chorioretinopathy (CSR). Mr Mathew was explained about condition and causes for same. He was relieved to hear that this condition is non-vision threatening and was asked to follow up after three weeks.
Central serous chorioretinopathy (CSR):- It is an eye disease characterized by leakage of the retinal fluid that has tendency to accumulate under central macula. Hence, central visual field is affected. This eye condition is said to worsen with stress. In most cases it resolves by itself. However, in some cases it may persist and becomes chronic. Depending on the location of the leaking point, either a focal laser or PDT (Photo Dynamic Therapy) can be done for the resolution.
Macula is a part, which concerns with central and reading vision, hence the condition, is visually very bothersome. Patient will present with blurring of central vision with reading difficulty. Condition is frequently associated with Acute stressful condition, lack of sleep and Type A personality. There is also strong association of this disease with Steroidal medication intake in any form like tablet/ointment/inhalation spray.

After three weeks, Mr Mathew’s visual acuity showed stability but the swelling was not reduced. So this time he asked to undergo a test called FFA (Fluorescein Angiography) for detection of source of swelling in Retina. When FFA test was done (refer below give picture of Mathew, it showed leakage (smoke stack shaped) at macula in his right eye.

Dr. Yogesh recommended him to undergo focal laser since the leaking point was away from the fovea. He then asked George to follow up after 15 days.

And after a fortnight of laser treatment, Mr Mathew was glad as he experienced significant improvement in his symptoms. His uncorrected vision improved to 6/9 and after 1 month it further improved to 6/6.
